ATTENTION PARENTS!!!!!!  New laws have been passed that will become effective in the 2009-2010 school year.  EVERY SIXTH GRADE student next year will have to provide an updated shot record to attend school.  Shots that are now REQUIRED for entry into sixth grade are the following: Tdap, 2 dises of Varicella, 2 MMRs, 3 Hepatitis B, and Meningococcal Vaccine (MCV-4).  The three shots that your child is most likely missing are the following: Tdap, MCV-4, and a second Varicella vaccine.  The Varicella vaccine is not required if your child has had the chicken pox disease, but history of the disease must be noted on the shot record.  Kindergarten (5 year-old shots)are unchanged except the addition of the second dose of Varicella vaccine.  Please make an appointment to get these vaccinations this school year, shot records will be due July 1, 2009.  Once your child has been vaccinated, please bring an updated ORIGINAL shot record to First Aid.

 

Dear Parents of Students at St. Thomas More School:

If your child takes medication at home, needs to take medication at school, has allergies, medical conditions, or any food restrictions, please contact the nurse to obtain proper medical forms. Medication at school can only be given if forms are signed and certain conditions are met. The STM Parent-Student Handbook contains our medication policy. If you have any questions or need forms, please call 275-9726. In the summer, the nurse works part-time, so please leave a voice mail and your call will be returned as soon as possible.
